The ordinary visa, stay certificate and residence certificate was lost, damaged or stolen after foreigner entered China，these situations can apply re-issuing documents by the below formalities:
1.Handing over the original of the valid passport (or other international travel certificate);
2.The original and the copy of the Registration Form of Temporary Residence for Visitors (issued by the hotel or the police substation of the area where the lodging takes place);
3.Submitting the Foreigner Visa and Resident Permit Application Form completely filled up and one recent photograph (2 and half inches length, bareheaded and full-faced);
4.Submitting the certificates conforming to the origin of application：
(1) If the ordinary visa, stay certificate and residence certificate was lost or stolen, the holder should hand over the certificate reporting the loss of passport or the official document from the embassy or consulate of his(her) home country in China;
(2) If the ordinary visa, stay certificate and residence certificate was damaged, the holder should present the damaged passport or the note from the embassy or consulate of his(her) home country in China;
(3) If the group visa was lost, stolen or damaged, the holder should hand over the certificate letter of local host travel agency and the copy of the group visa.
